Join Our Team as a Social Worker PRN at Houston Methodist!

Are you passionate about making a difference in the lives of patients and their families? At Houston Methodist, we are looking for a dedicated Social Worker PRN to join our compassionate team. In this role, you will provide comprehensive clinical social work, conduct psychosocial assessments, and facilitate complex discharge planning for a targeted patient population. Collaborating with physicians and the interprofessional health care team, you will help ensure that patients receive the quality care they deserve.

Your Impactful Responsibilities

• Effective Communication: Engage actively with healthcare team members, sharing vital patient care information and using therapeutic communication to build relationships with patients and families.
• Staff Education: Provide training specific to patient populations and department processes, serving as a mentor to new employees.
• Case Management Leadership: Lead comprehensive case management activities, assess high-risk patients, and identify barriers to efficient patient throughput.
• Conflict Management: Address psychosocial treatment plans, utilizing appropriate clinical social work diagnoses and interventions.
• Patient Education: Establish educational goals with patients and families, ensuring they have the resources needed for care after discharge.
• Quality and Safety: Document patient assessments and modify care based on continuous evaluation, focusing on psychosocial barriers to discharge.
• Financial Coordination: Assess unfunded patients and identify community resources to facilitate effective transitions.
• Growth and Innovation: Educate healthcare providers on community resources and lead performance improvement projects.

What We’re Looking For

Education

• Master's degree in social work from an accredited university.

Work Experience

• Two (2) years of social services experience in a hospital system setting.
• Healthcare experience is a plus!

Licenses and Certifications

• Required: LMSW - Licensed Medical Social Worker - State Licensure.
• Preferred: CCM - Certified Case Manager, ACM - Accredited Case Manager (ACMA), or LCSW - Licensed Clinical Social Worker.

Essential Skills and Abilities

• Strong communication skills to effectively interact with patients, families, and healthcare professionals.
• Knowledge of community resources and healthcare financial issues.
• Ability to work independently and exercise sound judgment.
• Proficiency in Microsoft Office Suite.
• Critical thinking, collaboration, and time management skills.

Working Conditions

• Work Attire: Business professional attire is required.
• On-Call: May be required to be on-call during emergencies.
• Travel: May require travel within and outside the Houston Metropolitan area.
